Exam: $10 copay
Frames: $40 allowance (10% off retail beyond $40)
Standard Lenses (single/bifocal/trifocal/lenticular): $10 copay each
Progressive Lenses: $60 copay for standard; $95/105/120 for premium tiers 1–4
Contact Lens Fit & Follow‑Up: $0 copay (20% off balance over $150)
In‑network: $40 allowance + 10% off retail over that
Out‑of‑network: up to $39 reimbursement
Lens types: single, bifocal, trifocal, lenticular, progressive (standard & premium Tiers 1–4)
